Solventum (NYSE: SOLV) raises outlook as sales fall and cash flow improves

What happened

Solventum Corporation (NYSE: SOLV) said third-quarter sales fell, but cash flow improved and management raised its outlook.

That mix matters because investors want evidence that the post-spin business can produce steadier cash, not just a cleaner starting point.

Sales were $1.9 billion, down 7.0% from a year earlier and down 2.8% on an organic basis. The company said the decline reflected the expected 630 basis point effect from advance orders in the prior quarter ahead of its planned ERP cutover.

Operating cash flow was $177 million and free cash flow was $97 million. Both were ahead of expectations, and both benefited from an IEEPA tariff refund.

Management said full-year organic sales growth should land toward the high end of the 2.5% to 3.0% range.

It also raised adjusted EPS guidance to $7.20 to $7.30 and kept free cash flow guidance at $200 million to $300 million.

Why it matters

Solventum's spin-off story depends on turning healthcare scale into durable cash generation.

This quarter helps on cash, but not on sales momentum.

Compared with the same quarter a year earlier, operating cash flow improved by $102 million and free cash flow improved by $119 million.

That is the clearest sign in the release that the company is moving more cash through the business after the separation work.

The caveat is that organic sales still fell and the cash gain was helped by an IEEPA tariff refund. The quarter does not show a clean operating turn on its own.

Cash and balance sheet

For the first nine months, operating cash flow was $215 million, down from $274 million a year earlier. Free cash flow was negative $32 million, compared with negative $42 million a year ago.

That means the quarter was stronger than the year-to-date trend, which helps explain why the company kept the full-year cash outlook intact.

GAAP diluted earnings per share was negative $0.07. Adjusted diluted earnings per share was $1.45.

The statutory loss and the adjusted profit show how much one-time items affected the quarter.

Cash and cash equivalents were $451 million at Sept. 30, and long-term debt was $4.56 billion.

Investors can use that balance sheet to judge whether the cash improvement can hold after the separation work and debt paydown.

Margins and business mix

Adjusted operating income margin was 19.9%, down from 20.6% a year earlier. GAAP operating income margin was 3.5%. Last year's figure included a gain from the purification and filtration sale, while this year's figure included restructuring costs.

The GAAP margin is not a clean read on operations, so the adjusted margin is more useful here.

Solventum said Advanced Wound Care and Health Information Systems grew, while Infection Prevention and Surgical Solutions and Dental Solutions declined.

That split matters because the portfolio is not moving in lockstep, even though management said separation is more than 90% complete and ERP cutovers are complete.

What's next

Solventum will host a conference call Nov. 5 at 4:30 p.m. Eastern Time to discuss results and fiscal 2026 outlook.

If management keeps pointing to the high end of the organic sales growth range, the raised outlook will look more durable.

If not, the quarter looks more like a timing benefit than a lasting shift in demand.
