What happened
The Cigna Group (NYSE: CI) raised its 2026 adjusted earnings outlook to at least $30.60 a share, up $0.15 from its prior projection.
The raise came in a quarter when sales grew and profit did not. Total revenues rose 6% to $73.7 billion from $69.7 billion in the third quarter of 2025. Adjusted income from operations was $2.1 billion, or $7.98 per share, against $2.1 billion, or $7.83 per share.
Shareholders' net income fell to $1.4 billion, or $5.47 per share, from $1.9 billion, or $6.98 per share. Cigna says the drop is mainly due to an after-tax gain on a sale in the third quarter of 2025 that did not repeat.
Cigna Healthcare's 2026 outlook for adjusted pre-tax income rose $50 million to at least $4.6 billion. Its medical care ratio range narrowed by 50 basis points to 84.2% plus or minus 25 basis points. Evernorth Health Services is expected to earn at least $6.9 billion pre-tax.

Where earnings rose and fell
The two big segments moved in opposite directions. Evernorth Health Services' adjusted pre-tax income fell 4% to $1.83 billion, and its pre-tax margin slipped to 2.9% from 3.2%.
Pharmacy Benefit Services drove the decline. Its adjusted pre-tax income dropped 22% to $765 million from $975 million. Cigna blames large client contract renewals and strategic investments, as it said before.
Specialty and Care Services offset part of that. Its adjusted pre-tax income rose 15% to $1.06 billion. Cigna credits organic growth in specialty businesses, including higher biosimilar and specialty generic adoption.
Cigna Healthcare's adjusted pre-tax income rose 15% to $1.19 billion, mainly on higher net investment income and better operating efficiency. Its pre-tax margin widened to 10.2% from 9.7%. The medical care ratio edged up to 85.0% from 84.8%, mainly because of U.S. Employer business mix.

Why it matters
OptimistFi has argued that Cigna's scale produces recurring revenue and stable cash flow, but only if operating margins recover. This quarter supports the first part more than the second.
OptimistFi's calculation puts adjusted income from operations at 2.9% of adjusted revenues, down from 3.0% a year earlier. In dollars, adjusted income rose just 0.4%. The per-share figure rose 1.9% because the weighted average share count fell to 263.8 million from 267.5 million.
Costs also moved the wrong way. The SG&A expense ratio was 4.9% and the adjusted ratio 4.7%, compared with 4.8% and 4.6% a year earlier. Cigna says the increase reflects investments to support long-term growth, partly offset by operating efficiencies.
Customer counts shrank. Total customer relationships fell 6% from Dec. 31, 2025 to 177.6 million, on lower pharmacy and behavioral customers. Pharmacy customers fell 5% to 117.7 million. Cigna says this reflects expected client transitions and lower membership from health plan clients. Behavioral customers fell 19% to 23 million after a government contract ended, with minimal impact on revenues and adjusted income from operations. Medical customers rose 2% to 18.5 million.
The balance sheet gave some support. The debt-to-capitalization ratio was 41.8% at Sept. 30, 2026, against 44.9% a year earlier and 43.0% at Dec. 31, 2025. Cigna repurchased 1.8 million shares for about $500 million year to date through Nov. 4.
What Cigna promised at Investor Day
At its 2026 Investor Day, Cigna introduced its Lead to One vision. It set long-term targets of 10%-14% adjusted EPS CAGR and about $50 billion in cumulative operating cash flow through 2030.
It also announced Accelerate to One, a multi-year $3 billion modernization and productivity initiative. CEO Brian Evanko said Cigna is pushing it to drive productivity, cut costs and support long-term earnings growth.
These are targets, not results. The cost-cutting goal speaks directly to the margin question, but this quarter's numbers do not yet show it at work.
What's next
Cigna scheduled a conference call for 8:30 a.m. ET on Nov. 5 to review third-quarter results and the 2026 outlook.
The outlook floor sets the next test. Cigna earned $23.55 per share of adjusted income from operations in the first nine months. Reaching at least $30.60 for the year requires at least $7.05 in the fourth quarter, below the $7.98 reported for the third quarter.
A fourth-quarter result at or above that level would hold the raised outlook. A further drop in Pharmacy Benefit Services income would weaken the case that margins are recovering. Cigna's exit from the Individual and Family Plans medical business, announced in April 2026, takes effect Jan. 1, 2027.
